Admissions Form(Reception ) - This online form is for parents wishing to apply to this school for their child to start school next September. If you would prefer a paper copy of the form please contact Heather Hodge at the school on 01995 603454. Applications should be received by 15th January.  For the purpose of admissions a map of the parish boundary is available by clicking here

Administering Medicine Form - If your child is ill and has been prescribed medicine that they will need to take during the school day then Form 3 MUST be completed and handed in with the medication to the school office. This can be found on the policies page or by clicking this link. Thank you.

Admissions Form - This form is for children wishing to transfer in from another school either within the county or moving in from another area. It is NOT for children wishing to start in reception class. If you would like to register your interest in your child starting in reception at a future date please contact Heather Hodge on 01995 603454 or email her at with your child's full name & date of birth, your name and relationship to the child, address and a contact telephone number. We will then ensure that you are sent the relevant forms from County Hall in the October prior to your child starting school.

If you're moving into the area, or changing schools, you'll need to complete this form.  The address you need to send the application to is at the bottom of the second page.  It must be completed for each child that needs a place.

Data Collection Sheet - The school needs to hold up to date contact information on all the children on roll. If your circumstances change, please let school know as soon as possible by making any necessary changes on this form and sending it through to the school office either by email or or by handing the completed form in at school.

Data Protection Statement - The school has registered with the Information Commissioner's Office on Data Protection and all office staff have completed data protection training. The school's data protection statement can be accessed here.

Annual Church & Garstang Visit Consent  - As you will be aware we often take children out on local visits in connection with work in school. Local means in the immediate vicinity surrounding the school in Garstang. Each time we do this we need your consent for obvious reasons. To make the organisation a little less complicated we ask parents to sign this overall consent form to cover any local visits at the start of every academic year. Such visits would be on foot and we would have the requisite regulation staff supervision and cover and a risk assessment would be undertaken. We do of course still inform parents if their child was going to be out of school, but with a global consent form there is no need to reply each time.

Holiday Request Form - It is a legal requirement for parents/carers to obtain the permission of the Headteacher before removing their child from school in order to take a holiday during term time. Parents do not have an automatic right to take their children out of school for holidays during term time and may be issued with a Penalty Notice (£100 per parent per child) if they do so without prior arrangement with the Headteacher.

Home School Agreement - We ask all children and parents/carers to sign this agreement on entry to the school and we sign it too. It is an agreement to follow certain guidelines to ensure communication between home and school. 

Online Payments using Parentpay

We are now using a payment service called "Parentpay" to collect the payments for our school meals, breakfast club, after school trips and school trips. Parents are able to see exactly how much money they have paid to school and how much they may owe. All payments can be viewed and tracked very easily. The payment system is very much tried and tested, it does not remember card numbers and is absolutely secure. From June 2016 Parent Pay have made their system easier with "My Account". All pupils have been issued with log in details. Please just ask at the office if you need a reminder.

If you do not have internet access at home - just call into school and we can make a PC available for you and also guide you through the process if you wish.

If you do not have a bank account to enable you to make online payments we can issue you with your own "Paypoint"card. By presenting the card you can pay in cash, by cheque or card at any store displaying the "Paypoint" logo. Our nearest paypoint stores are Sainsbury's, on Park Hill Road and the Co-op on Garstang High Street. As these have much longer opening hours than the school office you will be able to make payments to your account outside school working hours, at weekends or in the evening.
